Arranging the Enablement Service
In Medway, the enablement service is provided by a nominated care provider.
All referrals to the provider should be made in line with local processes and requirements.
The referral should contain information about:
- The date that enablement is required to start;
- The frequency of enablement visits required at the start of the plan;
- The duration of enablement visits at the start of the plan;
- The timings of the enablement visits at the start of the plan;
- Any specific risks to workers;
- Any specific skills required.
When the referral has been accepted you must provide the following information to the provider:
- A copy of the Enablement Plan;
- With the person's consent, a copy of the assessment;
- Information and contact details of anyone else involved in the plan (for example an occupational therapy practitioner).
It is the responsibility of the provider to contact the person to agree the specific details relating to how the enablement service will be provided.
When this is confirmed the provider should update you so that the Enablement Plan can be amended accordingly if required.
You should then contact the person to confirm:
- The frequency, duration and timing of initial visits;
- That the frequency, duration and timing of initial visits are subject to change as enablement progresses;
- The name and contact details of the provider;
- When the enablement service is going to begin;
- That the Local Authority will be monitoring and reviewing the progress of enablement;
- What the person should do if they wish to request a review or are unhappy with enablement; and to
- Answer any questions they have about the service, or provide any further information or advice that you feel may be beneficial.
You should also take the opportunity to reiterate that enablement is a time limited service and is regularly monitored and reviewed.
